Thank you for your interest in the services we offer. We have made our new client forms and a checklist available to make sure your initial office visit with us is as convenient as possible.

Pleas follow ALL of the steps below for your initial appointment:

• Make an appointment by calling us at 310-546-6500.
  
  
• Download the New Client Forms and fill them out before the appointment. If services are for a child, please fill out the Child Intake form as well.
  
  
• If you have insurance, download the Insurance Forms. Please note the Insurance Questionaire form - it must be filled out entirely before your appointment. Simply call your insurance company (number on the card) and ask the questions on the Questionaire.
  
  
• IF you have UBH or Pacificare insurance, you will need to also download and fill out either the Adult or Child 'Wellness Assessment'.
  
  
• Please download and review the Therapy Assessment, Disclosure Statement and Agreement, and Notice of Privacy Practices documents HERE.
  
  
• Bring your co-payment amount with you in the form of cash or check.
  
  
• Carry your insurance card at your initial appointment.
